Year 12 to Year 13
Your end of Year 12 (mock) result is usually a very accurate prediction of what you'll get in Year 13. Almost everyone gets the same grade +/- grade. A very small minority of students achieve +2 grades of progress, and a similar proportion achieve -2 grades of progress, but around 80-90% achieve the same grade.
If you want a better grade: you need to do more work than other students to overtake them. Nationally grades are done in 'rank order' - the top 21% achieve A* and the next 20% achieve an A, so results are a bit like a race to the top.
The national average in term time is 3-5 hours per week per subject.
If you didn't do this, then you need to make up those hours over the summer.
Work for the grade you want/need:
What a D/E? You need to be doing 1-2 hours per week per subject in independent study.
Want a B/C? The national average is 3-5 hours per week per subject.
Want an A/A*? You need to be doing more than 5 hours per week per subject in independent study, either at school or at home, on average. This is sometimes less for Further Maths students.
Over the summer:
What a D/E? In general a D/E student completes some of the paper tasks, and little other work over the summer.
Want a B/C? In general a B/C student completes all of printed/paper tasks, but little or no independent work.
Want an A/A*? These students always complete all of the printed/paper tasks, and seek their own work on top of any spliced papers.
You need to prioritise learning Y12 Pure. Leave applied.

Ideally, this should be differentiation, integration, trigonometry and binomial expansion topics.
Bicen Chapter Summaries (10-20 mins) are a good low-energy way to check for gaps if you don't want to do questions.
You should be preparing for MAT AEA STEP ESAT TMUA or similar, probably.
If not, I recommend OCR Papers.
